A serious incident must be documented in a written report that includes ten criteria.

Keep incident reports on file for two years while also easily accessible to Licensing.
When must a person complete training?
Each person must complete annual training.
May I count the certified lifeguard in the swimming child/adult ratio?
Yes if trained/authorized by Organization; no if not authorized to be alone with children.
Are animals allowed at my operation?
Yes, if all conditions are met.
What are the requirements for running hot water?
A thermostat must control the temperature; no higher than 120 degrees Fahrenheit.
How must I maintain a child's record that is not active?
The record must be available for review within 48 hours. Otherwise archived safe from damage.
How many hours of annual training are needed in a facility such as Seton?
20 hours annually.
May I use a trampoline?
Yes, with conditions.
What are the parameters for sinks used for food service or food preparation?
All sinks must be supplied with hot and cold running water under pressure.
What parts of my operation must be ventilated?
Living quarters, recreation areas, dining areas, and kitchens must be ventilated by at least one window/mechanical system.
How must I maintain personal records?
You must maintain annual training records for current personnel for the last full training year & current training year, with exception of maintaining personnel records for a year after employment or end of an investigation.
Operation cares for children younger than two years old, what additional components must be included in general pre-service?
Recognizing and preventing shaken baby syndrome, preventing sudden infant death syndrome, and understanding early childhood brain development.
Does licensing regulate higher risk recreational activities?
Yes, Licensing regulates all activities conducted by the operation.
When must I have a fire inspection?
1. Before initial permit and 2. at least once every 12 months
What are requirements for lighting?
All living quarters, areas to avoid accidents; you must not use propane, kerosene, or other flammable fuel.
Who must consent to the release of a child's records?
Unless releasing to authorized personnel, or with explicit written consent, records cannot be released.
Does licensing approve training resources or trainers for annual training hours?
No. You must ensure employees receive reliable training relevant to the population of children served.
May children in care use all-terrain vehicles?
They may not ride or operate a three-wheel all-terrain vehicle, however, a child 16-years or older may drive/operate a four-wheel all-terrain vehicle.
Where must I post the emergency evacuation and relocation diagram?
You must post the emergency evacuation and relocation diagram in a prominent visible location in all buildings.
What type of beds are not allowed for a child?
Triple deck beds, veil beds, beds with restrictive bedrails, or any cribs unless age appropriate.
