What do you need to do for a CCM member?

Pull up "Scheduling CHP Appointment" Google Doc. 

  1. Go to calendar in member profile

  2. Schedule appointment

  3. Choose:  CHP Follow Up (Standard)-Phone

  4. Write description (i.e. Member is CCM after Comp. 1 completion)

  5. Go to the interval tab in the top right corner.  Choose 60min.

  6. Ask Samantha Claxton so she can inform you of the proper CHP.

  7. Choose the assigned CHP.  You will see if the assigned CHP is available in that time slot.  If CHP is not there then you will need to go to another time.

  8. Once you find an appointment then hit the Allocate tab at bottom right.

  9. Check member’s profile in Commons to ensure appointment generated (may need to refresh).

  10. Be sure to add the CHP to the member’s Care Team in their Commons profile.  Also, be sure to add in handoff note:  appointment time in the Plan section, @CHP in the Notify Team members section.
