Whether the agency has initiated an investigation of the report, whether the agency is continuing to investigate the report, whether the agency is otherwise involved with the child who is the subject of the report, the general status of the health and safety of the child who is the subject of the report, and whether the report has resulted in the filing of a compliant in juvenile court

Referral source, other family members, neighbors, professionals who have contact with the family or people in the community, whose contact with one of the members may have given them knowledge that would relate to the family assessment
What are collateral contacts?
Immediate or impending danger of serious harm to a child's life or health as a result of the actions or inactions of the child's parents, guardians, or persons legally responsible for the care of the child
