Go through each assessment part for what BUBBLEHER stands for and how you will assess the mom.
B- breasts (soft, look for mastitis, no engorgement, presence of colostrum, assess nipples for cracks, inversion, redness or pain)
U- Uterus (check fundus height, should be around umbilicus 6-12 hrs postpartum)
B- bladder (palpate bladder, ask about voiding, is it painful, how often, do you feel you can empty entirely)
B- bowels (last BM, narcotics may cause constipation, fear of going)
L - Lochia (explain all three stages, if clots check to see if it is tissues, earthy odor, scant to moderate amount)
E- episiotomy/tears (REEDA)
H- hemorrhoids/dvt (size/pain,discomfort. - check for one leg swelling, heat, pain, redness)
E - Emotions (bonding/attachment, support network)
R- rest and relaxation
