This assessment is supposed to be completed with the annual care plan update.
What is the Social Determinants of Health (SDOH) screening?
A Member must be considered disengaged from Health Home services when the Care Coordinator has not had direct contact with the Member throughout "x" calendar months despite multiple attempts to reach the Member.

These dates should be measurable to the intervention that the client is working on.
What are Target Dates?
Once the Member is moved to DSE status the blank must be updated to reflect what activities will be conducted to search for the Member
What is Plan of Care?
This form is supposed to be signed and uploaded into CareManager upon completion of the annual care plan update.
What is the Health Home Member Acknowledgement Form?
While Diligent Search Efforts are ongoing, Care Management Agencies may bill for the Member at the Member’s enrolled rate if and only if, "x" of the Diligent Search activities listed on the Plan are conducted each month.
What is three?
This may not occur if the Member’s Plan of Care is not up to date by the end of the billing month.
What is Billing for Core Service delivery?
If a client has not consented to an MCO on their DOH 5055, this title must be contacted so they can notify the client's MCO of the status change to DSE.
What is Lead Health Home?
Supervisor is also accepted since that is who would likely be notifying the lead.
The Plan of Care must be blank-blank, meaning it is developed with input from the Member whenever possible and includes the preferences, services and resources requested by the Member.
What is Member-driven?
If the decision is made to discharge a Member because he or she is no longer eligible or appropriate for Health Home services or the Member is closed due to loss of contact, the DOH 5235 Notification of Disenrollment in the Health Home Program must be completed and given to the Member at least "x" days prior to the actual case closing.
