The screening tool for the one-step process and identifies families that are eligible for services
Eligibility is determined by 1) participant age, the participant must be 13 to 24 years of age (some exceptions for fewer than 13 yrs. old), 2) Must be expecting or first-time mother, and 3) must reside in the geographical area of services.
Once the referral is received, the FSSC will make the initial contact with the participant. The FSSC will ask a series of questions to determine if the participant is eligible for services. The FSSC will schedule a home visit and bring the appointed FSS to the home visit to introduce the FSS to the participant. The FSS will schedule the first official home visit with the participant and complete the intake on the first visit and the assessment on the second visit.

Unidos Formando Un Futuro uses the Parent Survey for the two-step process.
Screening result is documented and monitored in the Referral Tracking Log by the FSSC on a monthly basis.

The goal of the program Unidos Formando Un Futuro is to
ensure that ninety-five percent (95%) to one hundred percent (100%) of the first home visits occur prenatally or within the first three months after the birth of the baby.

previous or current CPS Involvement,
Self-Esteem,
Available Life-lines(possible Depression),
Coping Skills,
Stressors/Concerns,
Potential for violence,
expectations of Infants’ Milestone/Behavior, Discipline of Infant /toddlers/Child,
Perception of new infant, and Bonding/Attachment Issue.
