Examples of this type of data include vital signs, height and weight, or measurable pieces of information
What is Objective Data?
This is the first step of the nursing process where subjective and objective data is obtained.
What is the Assessment Phase?
These nurses participate in planning by assisting in the development of expected outcomes and interventions and use their problem-solving and decision-making skills as well.
What are LPN / LVNs?
This ethical principle is to act to promote the good of the client.
What is Beneficence?
These are the assessment techniques in order.
What is Inspection, Auscultation, Palpation, & Percussion?
This type of data is whatever the patient says, such as: "My head hurts real bad!"
What is Subjective Data?
The RN will carry out the interventions that have been established. Part of this step involves using clinical judgments to monitor the client’s progress toward achieving their goals.
What is the Implementation Phase?
This nurse systematically collects data, reviews information about the client, and communicates to members of the health care team.
What is the RN?
The client can refuse treatment if they so desire. It is also important to note that the client needs to have the mental capacity to act and decide on their own behalf.
What is autonomy?
This technique requires advanced skill, for the experienced RN. It is a tapping of the client’s skin with a short, quick motion to determine vibrations and sounds depending on the underlying structure or organ that is percussed.
What is percussion?
Laboratory Results such as CBC, CMP, LFT.
What is objective data?
This part of the nursing process in which the nurse uses problem-solving and decision-making skills to prioritize care.
What is the Planning phase?
This phase of the nursing process is outside the scope of practice for a LPN / LVN.
What is Analysis Phase?
This ethical principle means to treat everyone regardless of their ability to pay for treatment, their social status, gender identity, or cultural or religious background. The nurse should be fair and impartial when providing care.
What is justice?
This assessment skill uses the following tool- stethoscope.
What is auscultation?
RNs analyze the subjective and objective data collected during assessment using clinical judgment. Then the RN collaborates with the client to develop the client's plan of care, identifying both actual and potential problems. They consider the need for education as well as the client's readiness to learn and any barriers to learning. This step is important in determining nursing interventions, which will be necessary for health promotion. This step is outside the scope of practice for the PN.
What is Analysis Phase?
This nurse reinforces client teaching by providing educational materials to the client, allowing time for questions, and providing clarification of the initial teaching.
What is the LPN / LVN?
This ethical principle means respecting the rights of the client to maintain privacy.
What is confidentiality?
This assessment technique involves the act of using the dorsal surface (or back) of the hands and fingertips to feel areas of the body for various findings, temperature, moisture, or unexpected findings that may have been noticed during the inspection.
What is palpation?
The nurse will evaluate the effectiveness and achievability of the goals and the need for adjustment of the interventions.
What is the Evaluation Phase?
What is the RN?
This means to do no harm. It is first and foremost in health care.
What is nonmalficence?
During auscultation, this side of the stethoscope is used to listen to high-pitched sounds, including breath sounds, bowel sounds, and expected heart sounds.
What is the diaphragm?
