This starts when restraints are applied and stops when they are removed.
What is a restraint episode?

This type of behavior and/or restraint is applied for a patient that is biting, kicking, punching, throwing objects, and/or active suicide attempt.
What is violent behavior/restraints?
These can be applied to skin of incontinent patients.

These are implemented for all hospitalized patients to prevent falls.
What are Universal Fall precautions?
This type of behavior and/or restraint is used when people are pulling at lines/tubes/dressings; or threatens to dislodge medical devices.
What is a non-violent?
This should be done upon hospital admission, daily for the hospital duration, upon unit transfer, following any procedure greater than 2 hours and anytime a patient's condition changes.
What is a pressure ulcer risk assessment?
Fall within last 6 months, altered cognition, balance or gait disturbance, altered elimination, and/or sensory deficit.
What are patient-specific risk factors over and above the existing, extrinsic environmental risk factors?
Continuous monitoring is required in this type of restraint.

Nonblanchable erythema of intact skin, is seen in this stage of pressure injurly.
