When do you use an LMRA? Give at least one example.
At the start of a task to identify, evaluate, and address the worst consequence(s) that can reasonably happen, as well as:
1. After a Pre-Task Briefing or group discussion as the user considers what that information "means to me” and the task at hand
2. When work is stopped in response to a trigger
3. When conditions / circumstances change
4. When rushed, distracted, uncertain, or when something just doesn’t feel right
5. When returning to work after a break, a near miss, or an incident occurs
Name a Target Area from your teams FLS LEAD plan.
Team Supervisor/Lead to confirm if Target area is correct!
What is the difference between a Hold point and a Trigger? Give an example.
A Hold Point is considered a “planned interrupter” used to move us from Fast to Slow Thinking and drive us to pause work. A Hold Point ensures all required steps and safeguards are verified before an irreversible action occurs, like a stop sign at predefined stages.
A trigger is considered an “unplanned interrupter” used to move us from Fast to Slow Thinking and drive us to pause or stop work. Triggers are clear signs or behaviors that signal a possible hazard or an error-prone situation ahead.
What is the "Life Saving Rule" outlined On the Breaking Containment/OPE LSRA SWC?
"Manage ignition sources & exposure to energy & hazardous substances"
What are the 4 questions we ask when doing an AAR? (After Action Review)
1. Tell us what was supposed to happen?
2. Explain what actually happened?
3. Describe what were the differences?
4. Describe what we can learn?
What are the 2 objectives of PSMS?
PSMS Objectives:
1. Eliminate Fatalities / Life Altering Injuries (LAI)
2. While on the path to achieving that objective, deliver Industry Leading Safety Performance
Describe the meaning of Start Work Expectations.
Start Work Expectations means life altering injury potential has been recognized, we have engaged each other in a pre-task briefing and review of the applicable LSRA and Start Work Checks, and we have Verified and Validated safeguards. In addition, the FLS gives the green light to start work.
Describe a scenario where a JSA can be used for a job task.
1. When executing a task involving one or more LSRAs to supplement the SWC discussion in instances when there are task specific details or safeguards that are not addressed in enough detail in the SWC.
2. When executing a task that does not involve a LSRA but does have the potential for life-altering injury or a significant process safety event
What is the difference between a V&V of Safeguards and a TLO?
V&V (Verification & Validation)
TLO (Targeted Learning Observation)
A V&V of Safeguards is a proactive field engagement process used by Supervisors to ensure life altering work safeguards are in place and effective.
A Targeted Learning Observation (TLO) is a Planned field engagement used to observe how work actually gets done compared to a job standard. TLO's are completed on targeted tasks with LAI potential consequences.
Describe how your Supervisor will develop his/her LEAD plan?
LEAD (Layered Engagement & Development)
1. LEAD begins with the development of Target areas. The supervisor will engage with their team members to develop the target areas.
2. LEAD plan is simply starting to plan opportunities in your target areas to do V&Vs, TLOs and AARs.
3. Execute LEAD plan in the field by using either of the 3 engagement tools.
4. Learn and Improve - findings from those engagements
