HOW TO MANUALLY CONVERT A CONTACT TO A PUI IF THEY HAVE HAD A RECENT POSITIVE TEST If a Contact states that they have had a recent positive COVID-19 PCR or Rapid test result within the last 2 weeks of your interview and they DO HAVE CSS NEEDS?

Mark Initial Disposition as: Agreed to Call and Final Disposition as: Completed Interview, then submit the file. YOU DO NOT GO AND COVERT INTO A PUI until CSS Needs are completed.
Reasons to extend a contact's isolation period
What is fever within the past 72 hours, emergency symptoms such as chest pain, shortness of breath and bluish coloring of skin?

What is Case's school is out of state, or Pre-K-12 is out of county, daycare etc.?
PUI who needs to be transferred to another jurisdiction (Out of Jurisdiction=OOJ)
What is:
Complete the interview with the Contact
Add a detailed note to the top of the note area of the Contact file, detailing the following: symptoms and/or positive test date and type of test; other information that will help the OOJ County understand this Contacts situation. Also add that this Contact should be converted to a PUI immediately when the file is received in the other jurisdiction.
Copy the detailed note
Change the county to the appropriate jurisdiction but before the final transfer paste the same note from the top of the file into the OOJ Transfer note area
Transfer the Contact file OOJ
Contacts who report symptoms via SMS text messages should be called by a Tracer immediately.
What is:
The Tracer must call the Contact to see if the Contact made a mistake, or if they really do have symptoms. TRACERS MUST NOT JUST ASSUME THE TEXT MESSAGE WAS CORRECT. TRACERS MUST CALL THE CONTACT FIRST TO SEE IF
THE CONTACT MADE A MISTAKE, OR ACTUALLY HAS SYMPTOMS! IF THEY STATE THAT THEY HAVE SYMPTOMS, THEN FOLLOW THE PUI PROTOCOLS, if contact has CSS NEEDS, then add great Notes and CSS will convert to PUI when completed!

2. Mark "High Priority
3. Add note with specific needs, test date, test site and test type. Also include "this Contact needs to be manually converted to a PUI due to symptoms, afer needs addressed
My case is is a nursing home resident
What is note the employee spoken too, ask if in the facility or hospitalized. Mark facility notification as disposition on CC. Mark last question on interview as YES (all activities complete?). Take good notes! Fill out case investigation monitoring escalation form
I keep testing positive! I am going to be in isolation forever!
