Evaporation takes place when heat is added to a liquid. It means the water changes from a liquid to a gas. Think about the steam that comes out of a kettle. It is extremely hot. The steam quickly cools. It forms tiny droplets in the air. These tiny droplets are visible. They form the "cloud" that you see. When these tiny droplets hit a mirror, they cool more. They form bigger droplets you see on the mirror. We say the steam condensed to form water. The change of gas to liquid is called condensation. This change takes place when the heat is removed.
