Outreach , Outreach Worker
This may be used when a participant is feeling unsafe at a particular READI community site, or may have a family member who is a staff at the same site where the participant is receiving services. Currently, this protocol can only be used before a participant enters TJ
What is Community Transfer Protocol
Name the 3 referral pathways an individual can come from to participate in READI Chicago.
Community referral, Urban Lab referral, Corrections referral (parole, CCDOC)
The maximum number of months a participant can receive subsidized transitional employment, coaching, and case management services
What is Eighteen or Eighteen Months
This approach requires the potential and capabilities our participants possess, build on the positive factors each individual brings to the relationship, rather than focusing solely on what is missing. Simply put this approach helps you see the best in people.

The number of months that a participant can receive follow-up support after their 18 months to promote lasting pro-social behaviors and engagement with the workforce after READI.

Case Staffings, Hot Spot Mapping, Safety and Security (Morning/ Evening Routine), Referrals for participants (Housing, Heathcare)
Facilitates partner meetings, signs off on stage advancement forms, participates in site visits, Heartland's brand ambassador for READI....and all around good guy
Who is Community Project Manager or CPM
Examples of these include: securing space for programming, participants clocking in and out accurately, giving participants consistent, and constructive feedback on their performance and progress, including using the Daily Feedback tool.
