Transition Credit
Any of these amounts already paid towards Health and Dental at UPH will transfer to Health Alliance.

No changes can be made to this benefit.
Flex Spending
Transition credits are only available if you do this.
Stay on the same Carle plan to which you were mapped.
If an employee wants to elect the Enhanced Dental plan they must do this.
Change their election during the special enrollment period.
These are not included in the Carle PTO Accural Rates.
Holidays
During your 10 day grace period on your email, this is who you can send and forward emails to.
Carle and UPH email addresses
If you make no changes to your elections, you will have to have this to make any changes.
Qualifying Life event.
Transition credits will be viewable on these, in the form of imputed income.
Paystubs
Team members residing within this many miles of a Carle Hospital will be provided with a Carle Network of providers.
35 miles
PTO balances may not be visible in Kronos or on paystubs until this date.
April 14th 2023
Team members who have been approved for, but not yet received payments for Tuition Assistance through UPH will be required to submit these to Carle for payment.
Grades
This mailing will inform you or your option to continue UPH coverage.
COBRA
This is the amount of the Tobacco User Fee for plan members who use nicotine.
$25.00 per pay period up to $50.00
These credits will not continue when you transition to Carle.
UPH Well-Being Credits
Team members with a PTO balance over the Carle maximum will transfer the full balance and continue to accure PTO over the maximun until this date.
October 8th 2023 (6 months)
If you have refills remaining, you must present this to your pharmacy.
New insurance cards.
Team members employed at the time of transition with this FTE status will be eligible for coverage in 2023 so long as they were enrolled in coverage at UPH.
.4-.49 FTE
Transition credits are based on this.
Your current tier of coverage.
These challenges through Optimum Health will be available to earn points for a bonus. The current points will transfer to Carle.
Wellness Challenges
This is what will happen to a team members current PTO upon transition if they are a FTE status of .4-.49
It will be paid out.
Team members who work 16-29 hours per week can use Carle's parental leave within 3 months of the event, if they have a birth or adoption prior to this date.
