Traditions are basically "the rules" of meetings. Formal. They provide guidelines between relationships in 12 step groups, membership and global 12 step society. 

12 concepts are the business rules of 12 step groups. They are an interpretation of AAs world service structure. 

12 steps are a spiritual path to recovery, outlined in the big book of AA, that an individual goes through with a sponsor.
