What is Pending?
Value used at the completion of the PDP/QIP when the SPARK Coach and program leader(s) determine if the quality improvement goal has been met.
What is Evaluation?
This would be used anytime a program participating in SPARK coach support is referred to another Tier of support. If a program needs to start a new PDP and would like to use the same I-SAT data, the program is referred back to Coordinator for Tier Assignment.
What is I-SAT Internal Referral?
Used to document when an IVS has expired and the program is no longer on IVS; the account record is updated at this time to reflect ‘IVS Status’ = Not applicable
What is IVS Complete?
Coach attempts to contact the program leader and the outreach is unsuccessful; this can be used for each attempt, or as a final documentation of at least 3 different days/times using at least two different methods.
What is Coach Attempt?
In a Referral or I-SAT case, a coordinator/coach has made all attempts to contact and the provider has been unresponsive OR the coaching cycle was not completed as planned.

What is PTQ Rating Request - Advancement?
General contact was made with a program. Example: contact to schedule a visit; reschedule a visit; check in before a visit or after a coaching session, etc.
What is SPARK Contact?
In a Referral or I-SAT case, the coaching cycle has ended, or the I-SAT was not a valid submission.
What is Terminated?
Value used during the process of developing the PDP/QIP using the information gathered during assessment to create jointly agreed upon program development goal, the necessary action steps to achieve that goal, and the timeline for doing so.
What is Goal Setting and Planning?
PDP in progress has ended, and is no longer relevant, OR, Program is discontinuing services.
What is I-SAT Termination of Services?
Used to document action when IVS request is submitted to SPARK and forwarded to OECOSL
What is IVS Request?
Program has declined SPARK's offer of support in follow up to a partner referral.
What is Referral Declined?
In a Referral or I-SAT case, support was provided and the related coaching cycle completed.
