In the January 2024, Vol 109, Number 1 article, sarcoidosis was defined as this.

Multisystem granulamatous inflammatory disease that can affect any organ

100

Sarcoidosis can affect the central nervous system.  The article listed several examples of infectious and non-infectious causes that need to be in the differential diagnosis.  Name one differential for each.

Infection: TB, Brucella, Fungi, Amoeba, Toxoplasmosis, Schistosomiasis, Varicella, HSV

Non-infectious: IgG4 disease, CVID, Histocytosis, Amyloid, Rheumatoid noduels, cholesterol granuloma, heavy metals, toxins, drugs. etc.

Consensus guidelines recommend diagnostic approach including radiographic findings, biopsy-proven noncaseating granuloma, exclusion of other etiologies, and this.

Involvement of more than one organ system.

This percentage of people experience spontaneous remission after 2 years.

Approximately 50%

Coincidentally, Valentine's day this year is also this religious holiday.

First day of Lent

The etiology of sarcoidosis is unknown, but the estimated heritability is in this range.

60-70% based on heterogenous studies across populations.

200

Sarcoidosis can affect any organ, which makes this particular differential diagnosis one that needs to be considered.  It can be a pain in the rear.

Chron's disease

200

This is a common finding on radiographic imaging that raise the question of sarcoidosis.

Bilateral hilar lymphadenopathy.

In the US the overall age-adjusted mortality rate for sarcoidosis is this

This is the term used to describe when a clear piece of glass or plastic is applied over a cutaneous lesion to look for blanchability.

Diascopy - used to see if a lesion is vascular vs non-vascular.  Sarcoidosis lesions are yellow orange on diascopy.

These two populations are at increased risk of developing sarcoidosis.

Women (twice as likely as men)

Black people

Note: avg age of diagnosis is 44-56 y/o.  Rare in kids.

Tuberculosis (caseating granuloma)

This syndrome is pathognomonic for sarcoidsis.  It is characterized by bluish-red or violaceous nodules and plaques that typically appear over the nose, cheeks, and ears.  Can cause disfigurement of nose, nasal obstruction, and sinus/pulmonary complications.

Lupus pernio

Patient's with no symptoms or mild disease should be monitored and do not require treatment. Symptomatic patients or patients with higher stage disease on testing should start this treatment first line.

Oral corticosteroids (0.5-1 mg/kg/day)

Reassess PFT and chest imaging after 1-3 months of tx.

Wean slowly over 12 months is responsive to treatment.

Boston pharmacist Oliver Chase invented this Valentine's day staple in 1847.

Conversation hearts.  Invented a lozenge cutter that was soon used exclusively for candy after Chase founded the New England Confectionery Company (later known as NECCO). Mycobacterium

Propionibacterium acnes (P. acnes)

This can be caused by cardiac sarcoidosis

Note: it can also cause 2nd and 3rd degree blocks as well dysrhythmias.

Lofgren syndrome has a more favorable prognosis and is pathognomonic for sarcoidosis.  It has these 5 features associated with it (name 3)

Erythema nodosum

Uveitis

Bilateral hilar lymphadenopathy

Polyarthritis

Fever
