The ability to bathe self including washing, rinsing, and drying self ( excludes washing back or hair). Does not include transferring in/out of tub/shower.
What is SHOWER/BATHE SELF..
The ability to use suitable utensils to bring food to the mouth and swallow food and/or liquid once the meal is presented before the resident
What is EATING
The resident completed the activities by himself or herself, with or without an assistive device, with no assistance from a helper.
What is 3, INDEPENDENT.
Admission/Discharge Performance-if the activity was not attempted and the resident did not perform this activity prior to the current illness, exacerbation, or injury.
What is Code 09, Not Applicable.
The ability to dress and undress above the waist including fasteners.
What is UPPER BODY DRESSING
Ms. S has multiple sclerosis, affecting her endurance and strength. Ms. S prefers to feed herself as much as she is capable. After eating three-fourths of her meal by herself, Ms. S usually becomes extremely fatigued and requests assistance from the certified nursing assistant (CNA) to feed her the remainder of the meal.
What is Code 03 (PARTIAL/MODERATE ASSISTANCE) The CNA provides less than half the effort for the resident to complete the activity of eating for all meals.
The resident was admitted to an acute care hospital after experiencing a stroke. Prior to admission, he used a cane to walk from room to room. In the morning, Mr. C’ s wife would provide steadying assistance to Mr. C when he walked from room to room.
What is 2, NEEDED SOME HELP.
Supervision or touching assistance: if the helper provides VERBAL CUES or TOUCHING/STEADYING assistance as resident completes activity. Assistance may be provided throughout the activity or intermittently.

What is LOWER BODY DRESSING
Mrs. R had a stroke, resulting in a severe communication disorder. Her family members have not returned phone calls requesting information about Mrs. R’s prior functional status, and her medical records do not include information about her functional cognition prior to the stroke.

Mrs. L had a stroke one year ago that resulted in her using a wheelchair to self-mobilize, as she was unable to walk. Mrs. L subsequently had a second stroke and was transferred from an acute care unit to the SNF for skilled services
