This form populates whenever a meeting is created.  It should be sent to all IEP team members including the parent with at least two weeks notice (as a best practice).  Legally it just needs to be sent with enough time for the parent to attend the scheduled meeting.  

What is the Notice of IEP Team Meeting (NOM)form or what is NC 6A?

Located at the top of the Home Page, this section communicates to SIRAS users various matters that are pertinent for that time of the year.  There might be information about state testing in the spring or information about ELPAC testing in the fall.  There could be information regarding updates to forms located here.

This document should be provided to the parent at the beginning of every IEP team meeting and should be sent home with all documents related to an upcoming IEP (with the notice of reassessment, the assessment plan, the notice of IEP team meeting and any other prior written notice provided to the parent).  This document outlines the parent rights and provides information to the parent regarding who to contact if they have disagreements or concerns related to the process or procedures that are being followed.

This document outlines the assessments the district will complete prior to a students initial evaluation for special education or prior to a triennial reassessment.  This document should be sent to the parent and signed.  Once the signature is obtained the district has 60 days to complete the assessments and hold a meeting where those assessments are reviewed.

If the IEP team is unable to complete an IEP meeting due to time or other factors, an continuation meeting is scheduled.  This meeting notice is used to notify IEP team members including the parent of the scheduled continuation meeting.  It is important to use this form and not write over the previous Notice of IEP Team Meeting form.

This form should be used infrequently.  This form notifies the parent if an IEP team member will not be able to attend the scheduled IEP Team Meeting.  This form should be provided to the parent in advance of the meeting and parents need to provide permission to carry out the meeting with the IEP Team Member absent.  This form should only be presented at the beginning of the meeting when an emergency has occurred (illness, accident etc.) and the IEP Team was not able to notify the parent prior to the meeting.  Again the parent needs to give permission for the meeting to be held without the member present or the parent can request the meeting be rescheduled.

This button on the toolbar allows you direct access to the area within SIRAS where you can access the forms and develop an IEP for a specific student.  When you are within a specific student's record this button will display the meeting that is in development.  When you are on the home page or if there is no open meeting, this button will display IEP Manager.

Whenever a student with an active IEP transfers into a school district from a school district outside of the SELPA, the administrator of the receiving district or county office of education shall ensure that the student is immediately provided services for a period not to exceed 30 days. A meeting is not required upon enrollment.
