No. KMT 30128 - Only forms that are for the current or prior year effective dates can be accepted.

22003 - The WEL100 (or WEL400) Report is not to be sent to any outside entity. If the group's GA calls in and wants to know which employees have done the wellness check, they can be advised of the number of employees that have completed it, but cannot be told who specifically, or sent a report.
