Mary falls off from a ladder in her garage, lands on her elbow and hears a loud "pop". Her husband takes her to the emergency room where her xrays reveal a non-displaced fracture on the shaft of her humerus. Mary is placed in a splint and sling and given instructions to take tylenol for pain management. Mary's husband is upset and asking you why his wife was not put in a cast. Why wasn't she placed in a cast immediately? What do you say?
Mary will be reassessed in a couple of days, after the swelling decreases. If a cast is placed now her swelling could continue and the cast will become so tight that it cuts off circulation and causes severe pain, resulting in the need to return to the ER to have the cast cut off. OR -If the cast is applied now and the swelling decreases, the cast will not properly fit and places Mary at risk for not healing properly or further injury.
