The steps to making a DHS report are..

1. Call Hotline (1-844-264-5437)

2. Complete attached Report Documentation

3. Give therapist the original report

4. Therapist will provide the document to Monica

5. Monica will scan the document, email the document to Mary and Bumgardner and save the document to a Syncplicity file
