Level One Requirements
A conservatorship is a court-ordered arrangement where a judge appoints a person to manage the affairs of another adult who is unable to do so for themselves
Marketplace is when you are able to get snacks and drinks such as chips, candy, and soda. It happens Monday, Wednesday and Friday.
How many phone times do we have? What times are they?
3 phone times. 10am-11am, 1pm-2pm, 6pm-7pm.
How can you be LEVELED DOWN?
Not attending groups, physical/verbal aggression towards peers or staff, medication refusals and not completing ADLs
Level Two Requirements
How often is conservatorship renewed?
Annually
How often are outings? What level do you have to be on?
We have outings a minimum of once a week. You have to least be on Level Two to participate in outings.
How many times do you HAVE to shower a week?
A minimum of TWO showers must be completed. Otherwise, you may lose a smoke break or canteen privileges.

7PM. All residents must be inside by 7pm. If you would like to go outside, it must be with staff supervision.

Do I have to be off conservatorship to discharge?
NO - almost everybody who discharges is still on conservatorship. You are able to go to a board and care facility while on conservatorship.
Is smoking a RIGHT or PRIVILEGE?
DAILY DOUBLE
Smoking is a PRIVILEGE.

Lighters, matches, electronic cigarettes, vapes, cell phones, computers, caffeine, aerosol sprays, dental floss, sharp/metal objects, razors, glass, etc.

How do I contest or get off my conservatorship?
You must be able to have a viable plan for your basic needs (food, water, shelter, etc.)
What level do you have to be on to go on out of pass?
LEVEL 3
How many smoke breaks do we have? What times? How many cigarettes can you have?

5 scheduled smoke times: 8am, 10:45am, 1:15pm, 3:45pm, 6:45pm. You can have ONE cigarette.
What time do the morning and evening ADL's must be completed? Name 3 ADL's on morning or evening checklist.
Morning - 9am and Evening -9pm. Change clothes, brush teeth, make bed, use deodorant, check group schedule, brush teeth
