Bridge program is our weekend tool that allows us to admit a patient on a Friday. Once a patient is admitted, they will receive daily RN visits through Monday for that first weekend on hospice.

Hospice patients with high fall risks will receive a PT eval from outside PT agency. Role of therapy is to assist with transfers/fall prevention/implement additional equipment needs.

Do you treat wounds?
We do not treat wounds. However, St. Croix has a wound care program that allows us to partner with an outside wound agency. Their role is to provide us with a care plan to appropriately manage these wounds in an effort to keep patients comfortable.
