Using any color besides blue or black ink, using anything other than legal names, not completing the form in its entirety, scribbling through mistakes, using another child's name (breaking confidentiality), not knowing the difference between incident & accident reports, not reporting dual incidents, teachers not giving report because they are afraid of confrontation, not putting whole date & time, using shorthand/abbreviations, assuming child's feelings, leaving details out of report (icepack offered, teachers that were in ratio, etc.).

Nap mats must be 2 feet from everything, nap mat sheets MUST be tightly fitted to the mat, children must be put to sleep on their backs regardless of if they can roll over, red sheets must be used for our infants who may not roll over, children must not have hoods or strings on them, they must not go to sleep with anything in hand such as toys, lovies, books, etc., no sleeping with a bib on, if a binky comes out of their mouth while sleeping, it must be put away immediately, child must be removed from their mat & sleep sack as soon as they wake.
