What is the process for visitors to check in at my apartment?
Visitors to my apartment must check-in at the front desk and complete a Visitor’s Application. They must also provide a photographic identification card that will be kept at the Front Desk during the duration of their visit.

Will I commit to not abusing, destroying, or taking any facility property located in my apartment or onsite, and to reporting any issues with equipment or room furnishings immediately to management?

Am I responsible for securing any type of rental assistance, and will staff assist me as necessary?

Can I Share my apartment access card?
No, I agreed to not share my apartment access card with anyone.
Will I possess or use illegal or contraband drugs either in my room or on Housing First property?
I will not possess or use illegal or contraband drugs either in my room on or Housing First property.
Will I refrain from engaging in any profane, loud, disorderly, abusive, or illegal conduct or activity either in my room or on the Housing First property, with the understanding that staff will report any illegal activity to the police?

Am I willing to undertake my laundry and housekeeping, dispose of my trash, and maintain my room in a clean, sanitary, and neat condition, understanding that if I have a disability requiring assistance with laundry, cleaning, or trash removal, staff will provide the necessary help?
I will do my own laundry and housekeeping, take out my trash, and be responsible for keeping my room clean, sanitary and neat. If I have a disability to that requires help with laundry, cleaning or trash removal, staff will be glad to help me.
I am responsible for paying rent by the 5th day of every month.
Is staff responsible for my belongings?
No, I am responsible for my personal belongings. Housing First management assumes no responsibility for damages or theft of my belongings.
Will I refrain from giving medication to other tenants or taking medication from them?
Yes, I will not give medication to other tenants or take medication from other tenants.
How can I best cooperate with staff requests to ensure I receive the assistance I need?
I know that staff is here to assist me. I will cooperate with all staff requests, so that way the staff will be able to care for my needs.
Do I agree not to tamper with fire alarms, sprinkler systems, security camera equipment, or use unauthorized electrical appliances in my apartment, with the understanding that the use of electrical hair dryers and razors is permitted?
I will not touch or disturb any fire alarm, sprinkler system equipment or security camera equipment, and I will not use any electrical appliances in my apartment other than those provided by the program (microwave, mini-refrigerator) or those approved in writing by staff. I may use electrical hair dryers and razors.
Should I immediately notify staff of any change in my rental assistance or income level?
I will immediately notify staff of any change in my rental assistance or income level.
What is not allowed, for visitors to bring to my apartment during a visit?
My visitors may not bring alcohol or illegal drugs into the facility.
Will I refrain from possessing or consuming alcohol in the common areas of the Housing First property?
I will not possess or consume alcohol in the common areas of the Housing First property. I may have alcohol in my apartment for my personal consumption.
How can we ensure that noise is kept to a reasonable level at all times and that no one is audible in the common areas between 10 p.m. and 8 a.m.?
By keeping my noise to a reasonable level at all times and that no one should be able to hear me in the common areas, from my room between 10 p.m. and 8 a.m.
Will I ensure that no pets are kept in my apartment or on the facility premises?
I will not have pets of any kind in my apartment or on the facility premises
Should I meet with my case manager if I am unable to provide rent?
Yes, you should always meet with your case manager to see if there is a payment plan, or benefits that could be issued out to you to help you pay for your rent.
What type of problems or concerns should be brought up to staff immediately.
I will immediately report any safety problems or concerns to any available staff.
Can Smoking in my room be grounds for a write up, and an eviction warning?
Yes, smoking in my apartment is not allowed, and it will cause a write up, and a eviction warning.
Do I agree not to loiter or panhandle around the facility, as well as refrain from doing so in and around the neighborhood or surrounding businesses?
I will not loiter or panhandle around the facility and I agree not to do so in and around the neighborhood or surrounding businesses.
Do I understand and accept that staff will regularly enter my apartment to do well checks?
Yes, I understand that well checks are mandatory for all clients.
Do I understand that I have a landlord-tenant relationship with the Housing First Program and am subject to eviction for non-compliance of any lease provision?
