What is the process for visitors to check in at my apartment?

Visitors to my apartment must check-in at the front desk and complete a Visitor’s Application. They must also provide a photographic identification card that will be kept at the Front Desk during the duration of their visit.

Am I willing to undertake my laundry and housekeeping, dispose of my trash, and maintain my room in a clean, sanitary, and neat condition, understanding that if I have a disability requiring assistance with laundry, cleaning, or trash removal, staff will provide the necessary help?

 I will do my own laundry and housekeeping, take out my trash, and be responsible for keeping my room clean, sanitary and neat. If I have a disability to that requires help with laundry, cleaning or trash removal, staff will be glad to help me.

No, I am responsible for my personal belongings. Housing First management assumes no responsibility for damages or theft of my belongings.

Do I agree not to tamper with fire alarms, sprinkler systems, security camera equipment, or use unauthorized electrical appliances in my apartment, with the understanding that the use of electrical hair dryers and razors is permitted?

I will not touch or disturb any fire alarm, sprinkler system equipment or security camera equipment, and I will not use any electrical appliances in my apartment other than those provided by the program (microwave, mini-refrigerator) or those approved in writing by staff. I may use electrical hair dryers and razors.

By keeping my noise to a reasonable level at all times and that no one should be able to hear me in the common areas, from my room between 10 p.m. and 8 a.m.

Yes, you should always meet with your case manager to see if there is a payment plan, or benefits that could be issued out to you to help you pay for your rent.

Yes, I understand that well checks are mandatory for all clients.
