  • There are also Service Request forms found in Jarvis in the App Status area and the Book of Business area – the SERVICE REQUEST forms are the PREFERRED method that agents should use. The Service Request forms are reflected in PCH within 10-20 minutes if not sooner; whereas the Agent Forms can take up to 72 hours to be reflected in PCH.


  • The agent can also call the Agent side of Member Services using the PHD phone number and IVR of 5,2,2 for address updates, claims information

Is a replacement notice needed for every Medicare Supplication application?

  • No – only if the client is replacing a Med Supp or MA plan
  • Sometimes if the client is replacing an Employer plan, Enrollments will require a Replacement Notice – so if it is reflected as a pending reason, it must be provided
