This needs to be checked for key information (parent signature, social security number, etc.) and must have the homeroom teacher's name on the top prior to submitting. It is due to Mrs. Gaines as soon as you receive it.
What is the Free/Reduced Lunch Application?
You have an incident that occurred in your class but is not defined on the FMS Guidelines for Discipline flowchart. You are unsure if it is referral warranted or not, so you do this.
What is consult an administrator/counselor?
"Get out of my class and go the office!"
What is how NOT to respond to a student that you need removed?
Running in the halls is an example of an offense that warrants a referral.
What is False? This is a minor infraction and can be addressed with a reminder of the appropriate way to move in the hall.
The response if an intruder were to breach your classroom and you and your students do what is necessary in an aggressive way to protect yourselves or keep safe.
What is "fight" (as in run, hide or fight)?
Hold this up to show that you are missing a student during a fire drill.
What is the red drill card?
When students are being defiant or disrespectful, this is the first person to call on for assistance.
What is an administrator? (Not the SRO)
Verbal correction, Lunch detention and Time out, are all examples of these.
What are Corrective Efforts/Intervention Strategies?
Research shows that establishing positive relationships and ensuring students are engaged in class helps to minimize student misbehavior.
What is True? When students are actively engaged, there is little time to misbehave and students will do anything you ask of them if you have an established relationship with them.

What is QUICKLY and QUIETLY
True/False. Students should be surprised that they've received a referral when they are called to the office.
What is false? Inform students when they have been written up so that they are aware.
Your students are repeatedly yelling out in class without raising their hands, making it difficult to teach. You want the behavior to stop. You find that you need to do this.
