Because Meraked can only be violated when the good or the bad gets left in the filter/sifter (ex: ring in sifter), However, if there is no good, then there is no violation of meraked.
Question: I don't make my own medicine so this is a pointless decree - people don't grind up medicine anymore.

Answer 1: Alternative medicine is still very popular - so the decree isn't useless
Answer 2: A person will do other מלאכות necessary to take the medicine - you may not grind it but you might drive to the store and buy it
Answer 3: Aבית דין can’t remove a decree unless they're greater in number and wisdom than the בית דין that made the decree
