CBC/Purple tube or any tube without separator serum in it.
Where should the double check be charted?
On flow sheet but more importantly in Icentra. preferably in a different time stamp to see it easier for audits.
When should the sharps container be changed out and how?
When it reaches the indicated line (3/4ths full). Close the used one and get a new one for the biohazard room.
What should you do if a patient is interested in home dialysis?
Let Russell know or contact the home team.
Where do we send most of our labs and why not all of them there?
Ascend. Some patients insurance do not work with ascend and if we need results quicker we send to intermountain core lab.
When should heparin be scanned?
Just like all medications, heparin should be scanned right before administration. Think about how to do this in your routine.
The patient should have their access arm wash with soap and water and then cleaned with antiseptic.
Ture. Please encourage patients to wash their access arm before sitting in their chair.

What test is the post lab for and why is it important?
Kt/v It gives us a value that helps us determine if the patient is receiving enough dialysis. The value needs to be 1.2 or greater.
What is the daily staffing sheet and where do you send it at the end of the day?
The Daily staffing sheet or DSS is to help Amber make time card changes. So if you want called of hours or to mark that your shift should have extra shift pay you can put it here. you can also indicate if someone was sick and who covered. You can scan them to Amber at the end of the day. she is in the address book.
What do you do if a patient does not show up for treatment?
Contact patient and inform them of the risks of missing treatment (hyperkalemia and fluid overload). Offer them another time if able. Tell them to go to the ER if they have any issues.
Contact provider to inform them of the no show.
Chart it with a note and in the dialysis document section.

Which line do you pull labs from and why?
Arterial line. If you pull from the venous line you the blood will be post dialysis and "cleaner" than the patients actual blood.
What documentation is mainly needed for billing?
Pre and post weight. Pre and post time. hours of treatment which should generate automatically if the cell is active. Treatment type (Hemodialysis or Ultrafiltration)
What should staff do if there is a disaster requiring an evacuation of the building? For example a fire.
If it is safe, pass out the emergency baggies to each of the patients. Then go and help the patients that may need assistance to exit the building. Meet on the south side of the building in the parking lot.
