What is clarithromycin and CYP3A4 and history of QT?
HepB virus reactivation
Coadminister with atazenavir or rifampin
Use caution in patients with Child-Pugh class B- No liver impairment
What is Mavyret (glecaprevir/pibrentasvir)?
Initial Episode of something treated with valacyclovir
What is Valtrex- 1 gram BID for 7-10 days?
(2:1) IM/IV: 1.5-3 g q6h
IV: 3.375g q6h or 4.5g q6-8h (max 18g/day)
What is Unasyn (amp/sul) and Zosyn (pip/taz)?
Adjustments for gentamicin and tobramycin.
Dificid.
What is fidaxomicin- 200 mg BID for 10 days.
May cause nephrotoxicity, neuromuscular blockage and respiratory paralysis, ototoxicity

IV treatment for shingles
What is 10-15 mg/kg/dose q8h for 10-14 days?
(PO- 800mg 5x/day for 7-10 days)
Pip/Taz Warnings (2)
What is sodium content should be consider in patients with sodium restriction AND also increased nephrotoxicity when given with vancomycin?
No pregnancy!
Treatment dose in naive pts: 400 mg BID or 1200 mg qd
Treatment experienced pt: 400 mg BID

Pts with infectious mono develops an erythematous rash with this drug.
