Who sees concerns reports?
Jeff Hooks, Lyndon Parrish, Brent Brewer, the President. This means that RHM's only see reports if you forward them to their emails. PLEASE REMEMBER TO DO THIS.
When doing Health and Safeties and you find a candle in a student’s room, but they are not present, what form do you fill out?

Noise Complaints
■Go to reporting party’s room and see if you can hear anything or if there is video of the noise
■Head to the room that is being reported against and knock
■Explain to residents in the room that there are quiet hours and courtesy hours. Do not tell them who has put in a noise complaint about them
■Make sure to include all parties present and those you spoke to in the concerns report

When do you list yourself as the "reporting party?"
When there is no one to "pin" it on, you list yourself as the reporting party! So when you smell marijuana and you can't pinpoint it, this is when you'd put yourself into the report.
What for do we fill out if residents as for RA's to sit down to resolve issues and come up with solutions after roommate agreements?

Room Entry
■Never enter alone
■If not an emergency, schedule a time with the resident to enter the apartment
■If you suspect violence, drug use, or other dangerous situations, do not enter: call the RHM and do not leave the scene
■Otherwise, get another RA or RHM to accompany you
■Knock and announce yourself 3 times
■Key into the room and announce you are doing so
■Once inside, identify yourself again
■Do not open fridges, closets, or drawers but it is okay to ask the occupant to do so
■Document what you find and communicate violations with residents of the room as well as your supervisor

Who do I list for noise complaints?
Everyone you speak to during the incident! This includes the names of the people who reported it and the names of the people who were present when you knocked on the door. DO NOT JUST LIST ROOM NUMBERS. Lyndon does not know who "105" is.
If a resident just puts check marks on their Room Condition report when checking in, what do you fill out on the side when checking them out?
If there are damages in the room, or good.
Please make sure that you are just not putting check marks and that you are filling out the RCR entirely.
Body Spills/Bio-hazards
■Contact RHM on duty
■Avoid contact with a potential body spill or bio-hazard (always assume that it is a body spill until it can be determined otherwise)
■Isolate and secure the area
■Use bio-hazard kit to clean it up (WEAR GLOVES)
■Write a concerns report
■We DO NOT clean spills in suites unless the student is going to be hospitalized

When do concern reports need to be completed by after a situation?
Immediately after or by 8:00 AM
What information goes at the top of a Room Condition Report?
Building, Room number and letter, N Number, Residents name.
Loss of Electrical Power
Investigate for potential reasons for power loss, if personal safety is not threatened by this investigation (e.g. is it a single room or entire hall?).
• Collect essential information for documentation purposes (identifying information and incident summary) and look for any fire policy violations that may be the cause (e.g. too many items plugged into an outlet).
• Check breaker box if this is an option.
• Contact the RHM on Duty and Maintenance as needed.
• Submit a work order if applicable to the situation.
• If loss of power is prolonged, aid in any procedure that is necessary or that is asked of by the responding RHM.
• Follow up with the resident(s) involved to make sure items that are not permitted, if any, have been removed from the room(s).
• If loss of electric power is building wide, assemble at the Front Desk and connect with the RHM on Duty.
• Verify that no-one is trapped in the elevator.
• Submit a Concerns Report.

Where do Room Condition Reports go after Check Out?
RHM's Mailbox or Keyboard
Sprinkler Activation (if activated by being broken)
■Report to the scene
■Collect appropriate information
■Contact RHM on duty to inform them of the sprinkler, the location, and any noticeable damages
■If possible, isolate and secure the area so that water exposure is minimized
■Write a concerns report
