How is the "Date Removed" field used in the Waitlist module?
To indicate: the date the first appointment was attended, the date the client determined they no longer want services
When two clinicians are working together to provide a service, where do you add the second clinician, and under which role?
Care Team Assignment - Role will be depending on the program but usually Additional Service Provider
Clinicians use this Progress Note indicate that they are closing a file following service provided.

Client Relationships
Define "Cancelled by Client" vs. "No Show"
Cancelled by Client=client has notified CCMH that they are no longer going to attend their scheduled appt.
No Show=client did not notify CCMH that they were not going to attend their scheduled appointment
CCMH wants to register every child/youth who is referred for services regardless of the outcome, for this reason.
To understand: demand for services, who we are not serving, how long it takes to make a decision, etc.
If a referral is considered to be a High or Urgent Priority, you will need to be sure to update it here

This widget which is used to capture important information for all staff involved with the record. It is where you would add the allergies of a Caregiver in an Early Parenting program

What is the difference between "Referral Date" and "Decision Date"?
Referral Date = The date we've recieved a referral/request
Decsion Date = The date the status of the referral was decided
This field appears in the Waitlist Module but can only be edited throgh the Referral Module
