This section of the UCAT is to organize applicant/member's support information and to assess the sufficiency of the support, living arrangements, support when sick, someone to talk to too, pets, telephone contacts and social interaction with friends/family.

This is the name of the documented UCAT assessor when the Oklahoma Human Services nurse is completing the assessment.

The applicant/member response to the prescribed diet is selected from the drop-down box. Multiple select option for specific diets is able to be documented.

Applicant has rapidly deteriorating health conditions that are not well controlled or are rapidly deteriorating, has numerous unmet needs for services available only through ADv or NF admission, requiring a high frequency/intensity of medical oversight, requires immediate NF admission if needs cannot be met by other means. Requires fulltime care performed daily.

Documentation of the Applicant/Member _________ statement will be added to the UCAT following the Subjective Evaluation of the Health area. This is a new area added to the UCAT.

These are fields that are disabled throughout the UCAT. These are auto-populated areas to assist the Case Manager and the HCMN can disregard these areas.

Key information: baseline functional ability both mental and physical, needs for services, preferences, existing services, informal supports and all issues that would need to addressed in the service plan.
