What is Professionals in the medical field; doctors, nurses, EMT's, etc. Because they are experts and knowledgable in this field of study, the speaker would not need to dilute or rephrase the proper terminology.

What is: A judge. The speaker is appropriately addressing his audience- in this case it is a judge, with the proper title because of his status. He would not address someone other than a judge, such as a store owner or dog walker using this term, because it would not apply to them. Therefore, it would be inappropriate to the audience.
