During the CPT meeting I should...
Review the ICP with the team using Zoom screen share. By reviewing the ICP you make sure to include all of the required elements in your meeting.
Where do I put family partner work on the ICP?
Family partner work can be a task under the youth's goal, brainstormed options, or represented by a family goal.
How many pages should my ICP be?
There is no set number of pages. When you complete your ICP, ask yourself if you believe the caregiver would read all the entire document.
Can I have one goal on my ICP?
Sometimes
Creating one goal tries to cover everything depends on the identified prioritized needs and if the needs the goal is addressing are similar to each other
How will I know when the family has met the vision or goal?
Asking the family & team to scale the vision and goal at every meeting.
Asking the family how they will know when they meet the objective
Including on the ICP for every task a start/end date, and tasks status
What can I update or create before the meeting?
You can review the cover sheet, ground rules, vision, and the medication before the meeting and have those items typed into the ICP.
What about brainstormed options for family partner work?
When the family partner creates their family partner worksheet they can use all of the brainstormed option. It is important to document all of the brainstormed options for every CPT meeting.
How can I make my ICP shorter?
You are only required to have three meeting summaries on an ICP update. The remaining meeting summaries live in Care Logic on the uploaded ICP's.
You also can take off a ICP after one meeting; any team member that left, completed goal, completed objective, and a completed tasks.
How can I make it easier to write the update?
Have a system to take clear notes to help you remember what was discussed in the meeting.
How do I make sure to include all of the required ICP elements in my CPT?
During your CPT prep identify priority needs.
Use an agenda in the CPT to help meet all of the benchmarks and to help with your time management
Review the status of the tasks at every CPT meeting
Scale the vision & goal at every CPT meeting
Include time in the meeting to review the safety plan
When do I create the ICP?
Our program specs require the ICP to be created and uploaded to the health record within 7 business days.
Does the family partner goal, objective, or task need to be related to the family vision and goal?
Yes!
If the family partner objective or tasks you are writing doesn't connect to the goal consider creating a new goal.
If the family partner goal, objective, or task doesn't connect to the vision please review with the family & FP to update the vision or change the family partner item you are writing.
Should I review the ICP with the caregiver and team?
Yes!
Review the ICP with the caregiver and be willing to make any changes to reflect the family culture and preferences that don't change required ICP elements.
What is best practice for writing my ICP update?
ICC program specs require the ICP be completed within 7 calendar days.
Staff find the greatest success when they schedule a time in their outlook calendar to write the ICP update, complete the safety plan update, and complete the telehealth attendance sheet. The ICP update is easiest to write when you schedule a time close to the CPT meeting.
What type of family and team strengths do I include in the CPT and where do I put them in the document?
You want team members to give functional strengths to help the family reach the vision and goal. The functional strengths are listed in the team members section and for each goal.
How do I know if I captured everything from the meeting on my ICP?
Take the time to review the ICP with the family partner, family, and team members.
What do I include on a family partner task?
The family partner task should reflect the conversation at the CPT and be a task the FP agreed to complete. The tasks should also be part of their ongoing job duties. The tasks should allow for some flexibility for the family partner to further define on their family partner worksheet.
How do I write the goal(s), objective(s), and tasks?
Use language to describe the core concepts the caregiver will understand
Can I combine a CPT meeting and IEP meeting?
It is challenging.
Best practice is to have two separate meetings.
If you are combining meetings it works best if you plan with the person running the team meeting how you can incorporate required CPT elements into the IEP meeting.
What do I need to know about brainstormed options?
All of the brainstormed options should be listed on your ICP - it doesn't matter if you use them.
At least half of your brainstormed options need to be community/natural supports

Where do I keep the ICP?
When writing the ICP we recommend frequently saving. Because the ICP has HIPPA, you must save the document on your P drive through remote desktop. After the ICP is completed you need to upload to Care Logic.
What is the best way to represent family partner work on the ICP?
During CPT prep, talk to the family partner and to the caregiver about the family partner tasks.
Give space in the CPT to have the family partner and caregiver reflect on their goals/tasks.
Communication is key between yourself and the family partner to make sure what you write in the ICP gives the family partner enough flexibility to complete their work and their required worksheet.
Ask yourself, could a friend or family member understand this plan if it were for their child?
If YES - Celebrate!
If Unsure - Consider using some of the reviewed suggestion
Reviewing the ICP with the family and/or family partner is the only way to know if the family can understand the plan
Ugh! I don't like this formatting and I want to change it. Can I?
No, you are unable to change the formatting on the ICP.
The ICP on the common drive must be used for all ICP's. All of the elements on the "ICP with progress scales" must be completed.
