Assessments
-First three days: you will be going through many assessments and evals to determine your current functional abilities.
-Results of assessments will be sent to insurance who will give an "estimated" length of stay. Your LOS will determine your plan of care while here. LOS can vary-some will go home sooner, some will need extensions.

Team Conference
-Team conference occurs once/week. During this meeting, therapy, nursing, physicians, and CM meet to discuss progress towards goals and your discharge plan. Once the meeting is complete, your CM will update you.

-Diet is set by your physician
-Ordering: Nutrition Aide will take your order one day in advance.
-Choices: Two features daily (specific entrees and sides available all the time)
-Snacks available at any time of day

*Guest Trays: If a guest is present during mealtimes, they can order a guest tray free of charge. This will be delivered to the patient's room.
*Cafeteria: Hot lunch is served 12-1:30 (T, W, H). with grab and go items available daily.
Senior Leader Rounding
Each senior leader is assigned to specific rooms here at the hospital. They will complete daily rounds to ensure your stay is going well and to assist with any concerns you may have.
Case Manager Role and Responsibilities
-Order recommended DME for discharge
-Plan for continued therapy after discharge (HH, OP)
-Schedule appointments with your doctor
Therapy Frequency
5 days/week (M-F)
*On weekends, your schedule will vary. Typically you will get one day of therapy over the weekend and have a day "off", or you will have two days "off". This could change so be flexible!
Visiting Hours
Hours: 8am-8pm
-Please try to communicate with your family/friends on visiting after therapy sessions are over or in between sessions.
*Special Accommodations (Family training/conference, overnight guests)
Mark, Ready, Set, GO!
MARK: (3 days before d/c day). Care Team will provide education.
READY: (2 days before d/c day). Testing, equipment/supplies, Exercise Programs
SET: (day before discharge). Celebrate your progress!
GO: (Day of discharge). You're going home!
Outside Appointments
During your stay here, if you have follow-up appointments related to your reason for admission (ortho, neuro, cardiology, etc.) you will be transported to and from appointment.
-Unfortunately, we cannot guarantee visits to your previously scheduled appointments (PCP, dentist, eye doctor, etc) will be approved.
Expectations During Therapy
*YOU are your rehab leader.*
Your therapists will provide exercises, safety tips, compensatory strategies, and encouragement. It is your responsibility to work hard and keep going no matter how tough it is!
-Your participation is most important!
Support Services
Psychology Services available.
-If at any point during your stay, you feel as though you would benefit from psychological assist, we have a Physician on-call that will come out and help.
Post Discharge
-You will receive phone calls from our facility personnel (Case Management) to follow up and check-in to make sure you are doing well.
Post discharge
-We encourage you to continue working hard to reach your goals.
*Use your HEPs, remember your safety precautions, and be confident!

Therapy Goal
Our main goal here at Encompass is to get you HOME.
-You may not be 100% when you discharge, but we will make sure you are able to complete all functional tasks safely and comfortably.
Independence Day
-Call light: We encourage you to use your call light when needed. Please be patient with your nurses and techs!
-INDEPENDENT in room: Once you have progressed and therapy feels you are able to be IND in your room, you will go through a "testing process" to make sure you are able to complete all tasks safely. A form will be filled out and signed by therapy, Dayshift RN, and Nightshift RN. Once all requirements have been completed, your bed alarms will be removed and you will be IND in your room.
