 YDP’s must complete the Case Closure form and submit to the Program Manager.

• The Program Manager will review the form and reasons for closure. This allows for 

the YDP and Program Manager to have a conversation about options for the 

participant. 

• The Program Manager will grant approval or denial. A deny will be validated with 

a reason on the Case Closure form. 

• Closure date will be entered on form.
