1163 providers must have worked in long-term care settings between 01/01/2011 - 01/06/2012 and have completed all the required training at the time.

Providers who hold an active RN (Registered Nurse), ARNP/AP (Advanced Registered Nurse Practitioner), or LPN/LP (Licensed Practical Nurse) credential are exempt from all HCA training requirements as long as their DOH credential is in good standing.

Step 2: Review preferred language make sure it matches the language they are requesting the material in

Step 3: Re-confirm mailing address. If Requesting ND materials make sure address is not a PO Box. If it is get a physical address and include in the case.
