My patient just grabbed a blunt and begins to smoke during session. 

Discuss with patient the importance of not being under the influence during sessions and see if they are able to delay smoking til later. If not, discuss how the blunt is helpful to them and in this moment. It maybe the patient is anxious or having other feelings that may not allow them to tap into other coping skills. Perhaps reviewing those coping skills at this time maybe more beneficial.

Patient has been called 2 times after a no show and there is no response. What happens next? 

Patient is eligible for engagement. If admin is involved they will alert clinician. If clinician states do not put them on engagement, please be clear in your explanation. If wanting admin to reach out again, please provide a reasonable limit. Ex: one more call, or email, etc.

I am the first person to consult if your patient is having issues with his/her psychotropic medication.

Patient states they are not happy with their therapist or Psych NP. If yes or no give explanation. 

Initial step is not to discharge. Must speak with supervisor first to see if matter can be resolved. If not, will try to look for another therapist internally.

Patient stated they are running low on their psychotropic meds prescribed by Dr. Goins and it is not a Thursday. 

Ask patient which medication, how much they have left and how is it prescribed. Notify BH Director via TE marked urgent.

TE is sent to provider from admin that patient is now on engagement. What is the turn around time for a response? How about if it is a discharge? 

48 hours turn around time for all TE's that are not considered urgent. 24 hours if urgent. Engagement is 48 hours and discharge is 1 week as there has to be a summary attached.

Patient just had a baby at the Birth Center and is going on maternity leave for 30 days and states that she has too much to do to attend therapy appointments. If yes or no, give an explanation. 

Initial step is to not discharge as maternity leave is a reasonable request. However, patient is a patient at COH and therefore has to come in for her postpartum appointments. Perhaps some brief check-ins can occur before or after those appointments. This way the patient feels supported but not thinking of separate times or an hour long session.
