What Communicates information to the donor about the procedure?

Donor Display

A blinking (*) to instruct the donor to assist blood flow by (pumping) opening and closing their hand.

An (X) to instruct the donor to relax his/her hand and not assist with blood flow.

How long can sets stay on machine?

Set ups can stay on machine for 4 hours once pumps have been loaded.

What are the steps to take if a donor does not receive saline?

If a donor does not receive saline, we will first document it on the machine after ending the procedure that way we can work on that worksheet. Then give the donor an electrolyte of a minimum of 250ml. We will then have the donor wait for 5 minutes. Set a timer. After that we will log into BECS and create a general issue of no saline for the donor and we will include the Collection number, donor number and visit number. Document what happened and that the donor left in good health.

100

What is the difference between an initial VP vs a 2nd VP?

The difference is when performing a 2nd VP. You will need a 3rd stat and ALWAYS bleed your line. You don't want to have any air in your line when you are doing your reprime.

A donor who is donating is starting to feel these symptoms, what type of DAE would this be?

*Lightheaded      *Pale

*Nauseous          *Sweating

*Dizziness

This would be a Hypotensive-Minor. We would

*Start a timer for 10 min (note the current time)

*Elevate feet *Check VP site for hematoma

*If donor is on a draw, put them in a normal return *Give them and ice pack or two

*Give them an emesis bag *Do not leave their side *Reassure them *Ensure reactions is not progressing, then create a collection exception

200

Acts as a safety valve, if air is detected early in DLAD1 & DLAD2, the procedure will automatically pause, and the valve will clamp closed to prevent air bubbles from reaching the donor, what am I?

DONOR DOOR VALVE.

200

Why do we stat our lines before removing our caps?

To Prevent Air contamination on our set up. We never want any air to enter the setup. It could cause air bubbles in our set up and could potentially give our donor an embolism.

200

What are the allowed limits for an overdraw and under draw?

Under draw: if more than 5 ml less than the nomogram collection volume.

Overdraw: if more than 5 ml over the nomogram collection volume.

200

What is the correct arm scrub size for Alcohol Swab stick?

It it 2.5 inches by 2.5 inches from the VP site.

You will scrub using vigorous back-and-forth strokes for a minimum of 30 seconds as indicated on the time. At the end of the 30 second "Swab Arm" time, the device automatically starts the "Swirl' timer for 10 seconds. You will then open a new alcohol swab stick. Move gradually outward in concentric circles, without overlapping any previously scrubbed area, while assuring the total prepped area is free of gaps. The total prepped area measures at least 3 inches.

200

What solutions can be used to clean the NexSys machines

Cavicide wipes, distilled water with lint free gauze or approved cleaning solution.

300

What moves AC solution from the solution bag to the disposable set where it mixes whole blood from the donor.

Anticoagulant (AC) Pump

300

What is the importance of raising the saline bag 18 to 24 inches above the machine, removing the stat, and gently squeezing the bag prior to hanging the bag on saline pole?

To ensure saline's proper flow but never passed the saline valve.

300

What are Post Donation Care Instructions?

It is the instructions we tell the donor after their donation.

  1. Keep wrap on the venipuncture site for at least 2 hours

  2. Keep the area dry and clean

  3. Don’t touch or pick at the site

  4. No heavy lifting

  5. Stay hydrated; drink plenty of water

  6. Eat a good meal

  7. Report back to the center if you're feeling unwell or if you have any trouble with the venipuncture site

300

How to identify an appropriate venipuncture site?

Inspect both arms to select a venipuncture site that is free of any infectious skin disease, tattoos, and is visibly clean and free of contaminants.

300

Can we leave biohazard waste or sharp containers in the areas overnight?

NO, you should always put your waste at the end of the day in the biohazard waste room along with your sharp containers. Sharp containers should never be past the filled line, if so, lock it and grab a new one.

400

The rubber gasket located in the bottom of the centrifuge chuck that holds the bottom of the disposable bowl in place, what am I?

O-Ring

400

I'm doing a set up, but as I'm inspecting my harness it looks like its missing one stopper, it should always be four. What are the proper steps to follow with my defective soft good?

I will first take my defective soft good and place it in a zip block bag with the wrapper it came in. Go and let a member of quality know about soft good being defective. Once I've reported it, I will now log into BECS and create my equipment issue. I will simply put the lot number, who I reported it to, who it was discovered by and a description of what was wrong with this equipment. Once that's done you submit finish.

400

How do you ensure there is no visible RBC loss prior to touching the set?

Observe tubing between the bowl and Donor line/ Plasma valve, to assure all red blood cells have been returned to the donor. Inspect your bottle to see if there's any RBC sitting on top of bottle line.

400

Where are your 2 stats placed before performing your VP?

One stat is at the bottom of the needle line. The second stat is below your y connector.

400

How do we perform our monthly's on the NexSys machine?

*Pump rotors and wells, we will unscrew them with a screw remover and clean with a lint free gauze and distilled water.

*Air filters, we take those out and rinse them in warm water. Once they are clean, we sit them to air dry and put a new clean filter (should be in the equipment repair room)

*Lubricate the centrifuge O-ring with silicone vacuum grease, you want to grab the slightest bit, comes a long way.

500

What does the Blood pump do during a draw?

It pulls anticoagulated whole blood through the blood filter and pumps it into the bowl into the centrifuge.

500

What are the time limits for the disposable set and AC/Saline? When must a setup be used after spiking AC and Saline solution?

*Saline and AC have 8 hours if CAPS ARE STILL ON once out of wrapper.

*Disposable set has 24 hours if CAPS ARE STILL ON once out of wrapper (bottle, bowl, harness)

*Once AC and saline have been spiked you have 4 hours to use set, if not used within the time frame, machine will time out and you'll have to waste the whole set up.

500

What are possible reasons for an overdraw?

  1. Device Failure

  2. Incorrect volume selection

  3. Incorrect donor height or weight

  4. Saline contamination

500

What is the cuff pressure set to while palpating?

We should set the cuff to 80. If they have good veins you don't need to set it that high.

500

How often do you check the biohazard waste bag attached to the machine?

Daily!!
