When and how long are home visits?
Once a week scheduled day. Each home visit is 90 minutes.

Who initiates the Transfer / Transition communication and to who and when?
The site receiving a child will initiate the conversation with the site has the child, Disabilities MUST be included if child has an IEP/IFSP . This should happen BEFORE the change.
Site /Program Option with child will initiate conversation with Family and coordinate with receiving program dates and meetings.

When a child is transferring, what income paperwork is needed?
Trick Question: no new income is needed for TRANSFERRING. FCCC/FSS/HB only needs to submit new Selection Criteria Points and SCF to ERSEA. Receiving staff may require proof of full time work/school/mixture/ or Seeking Employment Plan to justify full time care.
