Why would near fall points would not be scored on the edge of the mat?

To start the third period, the wrong wrestler is given choice of position. During the ensuing wrestling, Wrestler A is injured. He ends up using all of his 1.5 minutes. During this time A's coach points out to the referee that the wrong wrestler was given the choice to start the period.
What is"Bad Injury" time?

A coach calls the referee to the table. During the ensuing discussion the coach raises his voice and puts his finger in the referee's face.

The defensive wrestler B, stands, turns and grabs offensive wrestler A's leg. They go off the mat in that position.
How would the defensive wrestler gain an escape while not being totally separated from the offensive wrestler?
During a tournament match that ends in regulation time draw, Coach A questions the score. The Referee and the Assistant confer. The Referee then tells the Coach A that the score is correct. The Referee then starts the overtime period. After the start of wrestling the Coach again approaches the table. The Referee and the Coach discuss the score again and end up agreeing the regulation score was incorrect. Wrestler A should have won the match in regulation.
Is this a correctable error?
