What is the "support services status form"?
The support services status form is a document for HRS and Therapist cases that are not actively working on housing or therapeutic related goals. This document indicates that the case is "inactive", and pauses service plans/treatment plans and service summaries.
The HRS and Therapist still much reach out for a monthly, quarterly, or as needed check in with the client, which can be either via phone or face to face.
