What is Isolation
Being within approximately 6 feet (2 meters) of a COVID-19 case for a prolonged period of time (at least 15 minutes cumulative over 24 hours or consecutive); close contact can occur while caring for, living with, visiting, or sharing a healthcare waiting area or room with a COVID-19 case.
What/Who is considered a Contact?

What is the disposition?
Everyone must do this within 5 hours of starting their shift. This is a state law, not a rule. HR needs this compliance otherwise they can get fined and then they will mandate this.

What is 211?
When entering a new contact onto NBS, we want to be careful to check NBS for any existing record of a contact prior to adding a new entry into the record. These should be reported to your CN.
What is a Duplicate?
Ip's are ineligible if they are under 2 years of age, currently hospitalized, only have a P.O. Box # for address, or they are not a VT resident.
What is the Pulse Ox Program?
If an IP is in a group home with multiple residents, such as a correctional facility, a homeless shelter, mental health facility, dorm, or something similar.
What is a Shared Facility?
Obtain as much information as you can about this individual including their DOB, Address, phone number, and name. It is a good idea to have the IP spell the name if there is the potential for interpretation (“Katy” vs “Katie”). Furthermore, it is important that we obtain the individual’s legal name (“Katy/Katie” could be a nickname for a “Katherine”).
What is a Contact?
-Extra guidance for isolation or quarantine parameters
-COVID testing questions: test type, antigen vs. PCR, rapid vs. not
-If a case or contact had other COVID-related questions beyond the scope of the CI/CT and CNs knowledge
-Providing any additional resources for testing centers, vaccine locations, getting vaccinated while pregnant etc.
-School-related questions (if a parent believes their child was exposed, a child was a confirmed exposure/positive test, attendance of large group settings with high transmission rates, etc.)
-Questions regarding vaccines (what is considered fully vaccinated, can you still get COVID if fully vaccinated, meeting the criteria, etc.)
-Questions on if someone is outside of isolation period and continuing to test positive
-Interpreting results (confirmed vs. probable)
-Meeting outbreak definition

Other health services are available for an IP if an IP belongs to a particular affiliation.
What is Tribal affiliation?
For ALL Minors:
Thoroughly document all the exposure dates in NBS. Add additional information in the Supplemental Notes, clarifying the exposure dates, name of facility minor attends, and location of this facility.
What is School Exposure? (For Students)
TRACE MM/DD: Contact information obtained from PARENT/GUARDIAN, name, quarantine education provided, based on quarantine end date MM/DD. Any additional information that could explain exposure dates, contacts that shouldn’t be contacted, facilities they visited etc. M.Wilcox.

What are School Names?
You should be checking these daily (or when you have down time from cases) Once you have double checked each case, you then send over the IP's # and let your CN know you have completed this.
What is Open Investigations?
During an Initial Interview, an IP may need resources to help them to isolate or quarantine.
What is Care Coordination?
This section in NBS would be filled in when an IP has attended a large gathering or played in a basketball game during their infectious period.
What is Potential exposure- Community and Other.
When a quarantine period has to be extended past 14 days due to this.
What is ongoing HH exposures?
This should be used in a template format. You can add additional information to your template, (such as what school, special circumstances, anything that would be helpful to the DOH reading these cases.) But you must use this form at all times.
