Obtain as much information as you can about this individual including their DOB, Address, phone number, and name. It is a good idea to have the IP spell the name if there is the potential for interpretation (“Katy” vs “Katie”). Furthermore, it is important that we obtain the individual’s legal name (“Katy/Katie” could be a nickname for a “Katherine”).
