What is Dialectical Behaviour Therapy (DBT)?
An on-call mobile crisis service for older adults (65+) with addictions issues or mental illness. That will meet with the senior in crisis within 24-48 hours, either in their home or in their local community. Short-term crisis response is available 365 days a year from 9am to 5pm.
What is Crisis Outreach Services for Seniors (COSS)?
The standardized, comprehensive tool used in home and community care to evaluate the health, functional, and social needs of older adults and disabled individuals, helping providers offer tailored, independent living support through core questions and optional supplements (Functional, Mental Health, etc.). It's designed to track needs across care settings, identify early decline, and support person-centered care planning with input from the individual and caregiver.
