What are Community Support Services for Seniors?

Toronto. We work with people 16+ who are in need of mental health and
addictions supports, including counselling for their loved ones and
caregivers. We support older adults 55+ with a wide range of services from
food delivery to home care, exercise classes, opportunities for social
interaction and more.

What is the Psychogeriatric Assertive Community Treatment Team (PG-ACT)
This program offers social, recreational, and exercise-based programming that is available to anyone receiving services at Reconnect. Programming is offered both virtually and in-person.
What are the Health and Wellness Services?
Technique developed for clients with emotional dysregulation using individual 1:1 coaching and group skills training classes to help people learn and use new skills and strategies to develop a life that they experience as worth living.
What is Dialectical Behaviour Therapy (DBT)?
An on-call mobile crisis service for older adults (65+) with addictions issues or mental illness. That will meet with the senior in crisis within 24-48 hours, either in their home or in their local community. Short-term crisis response is available 365 days a year from 9am to 5pm.
What is Crisis Outreach Services for Seniors (COSS)?
The standardized, comprehensive tool used in home and community care to evaluate the health, functional, and social needs of older adults and disabled individuals, helping providers offer tailored, independent living support through core questions and optional supplements (Functional, Mental Health, etc.). It's designed to track needs across care settings, identify early decline, and support person-centered care planning with input from the individual and caregiver.
