This form is a graphical representation that shows all the systems at play in an individual's life including fictive kin. The FRS uses this form to identify resources and child and family support system. This form is used at the second appointment with the family and reviewed periodically with them throughout services as supports are added.

Quarterly, each network provider is to conduct at least one review per FRS.

During this phase, the FRS sets the stage for services. The FRS provides introductions to both him/herself and the program goals and services.  A schedule of sessions and the purpose for the meetings is reviewed, providing an orientation to the family about the program and expectations.

Quarterly program directors and supervisors conduct a quality check in with current families receiving services. This review is another mechanism to ensure that services are being rendered as designed and that families are satisfied with the services being provided.  Conducting this review during active status of service delivery allows for corrective action to be taken if needed.

This tool identifies goals and tasks that the family wants to achieve by the time of discharge from services. It utilizes information from the assessment process, referral concerns, and observations.

This form provides the family with a listing of their rights and responsibilities while receiving services as well as the grievance procedures.

This phase of the practice model provides an opportunity for the FRS and the family to reflect on the work that has taken place and the progress and the achievements that were accomplished. This phase also looks towards the future and devises an aftercare plan as needed. During this phase a chance to say goodbye and express the myriad of emotions are explored. Families are encouraged to complete satisfaction ratings and provide feedback to the FRS.

This form is used to document any contact and/or activities that occur for or with the family for service coordination.

Through a randomized process, the manager at the network provider agencies interviews the parents by phone or in-person. The responses to the interview questions are documented in Relias. Quarterly, each network provider is to conduct at least _____ family service quality check-in review per FRS.
