A patient expected to return to the home health agency is transferred to an inpatient facility for 24 hours or longger (for a reason other than diagnostic tests)
Transfer to an Inpatient Facility (TRN)-patient discharged from agency
A patient is transferred to an inpatient facility for 24 hours or longer (for a reason other than diagnostic tests) and is unexpected to return to the home health agency

Resuming a patient's care with the agency following a qualified inpatient stay

A major decline or major improvement unexpected in the patient's condition warranting an update to the plan of care
