True or False:

Nurses shouldn't be rooming patients, that is not their job and they should be relieved of ever having to room in the clinic, ever, ever.

False =)

There is no such thing as "that's not my job." We are a team.  Nursing will always try to make sure to watch if CNAs are struggling to room patients in a timely manner, but CNAs are also responsible to reach out to nursing if falling behind.  The patients in the clinic are the number one priority

List two times a result can be normal, but nursing will still need to call the patient.

-Any type of chest imaging

-Negative for malignancy

True or False:

If I don't know how to do an urgent, it's safe to assume someone else will take care of it.

False.

If you don't know how to do an urgent, the ideal solution is to find someone who does, take notes so you know the next time, and work through the urgent together. This is not always possible! If everyone else is busy, it definitely is okay to move to the next one and come back to it at a less busy time when you are able to ask.

List 3 examples of when we can and should send out a normal letter

Bonus: List a time when there are results that do not need a call or a letter.

-Stable ONO
-Normal labs
-Normal PFT and/or 6mwt
-Stable labs

Bonus: Stable labs that were drawn for procedure OR visit is scheduled within the next ~1-3 days (ensure pt attends visit before closing).

True or False:

If running down for a quick appointment, it's okay if I don't clock out.

False.

For any appointments, whether they are going to be long or short (within or outside HaysMed facilities), you must clock out.

You must also clock out any time you are leaving HaysMed facilities.  If you are leaving campus, but are back within 15 minutes, do not clock back in until you are done with your lunch ensuring you get at least 30 minutes.

True or False:

If a patient states that nothing has changed since the last time we have seen them, it's okay to mark medications off.

False.  We still need to go through the medication list with the patient and ensure the patient didn't stop something or start something new they forgot about.  Many times, patients will forget they finished an antibiotic/round of steroids/or acute pain management regimen.

200

True or False:

If it is downtime and I have my tasks completed, I can use my work computer to search the web.

False.

Your work computers should not be used for Pinterest, shopping, searching for new homes, or anything else besides work-related tasks.

200

What are three incidents a message should be marked urgent?

Possible answers (not all included)

-PET scans (scheduling, if pt cancels, PAs, etc.)

-Oxygen/Time-dependent orders

-Symptoms that were not marked as triage

-Upcoming procedures/clearance for procedures

True or False:

If we used to take care of a patient and that patient expires, I can look into the chart to see what happened.

False. Unless it is relevant to the care of the patient or information the provider needs, this is a HIPAA violation and ILLEGAL/termination-worthy offense.  While you may be curious and genuinely don't mean harm, it is still wrong, both ethically and legally.

If rooming a patient and the patient falls, what are the appropriate steps?

1. IF safe for BOTH parties, assist pt to the ground.

2. Ensure environment surrounding patient is safe/you are able to call for help from a nurse or provider.

3. Get a nurse or provider.  The nurse will likely be first available to assess the patient. The provider should also assess the patient. If patient refuses to be assess in the ER, patient needs to sign a refusal form.

4. Fill out a verge including location, patient, nurse, and provider who assessed.

300

True or False:

It is illegal to not take a lunch for consecutive days.

Both true and false.  There shouldn't be multiple times a week without taking a lunch. This isn't illegal on your end, but is on HaysMed's end. If you are not taking a lunch, you need to be putting in the comments why not because these are considered workers' rights.  If you are continuously not taking lunches, meet with your manager ASAP to change the rotation to ensure everyone is getting a lunch

Who is responsible for urgent messages?

A) Triage nurse

B) Nurse without an assignment

C) PA/Shot Nurse

D) EVERYONE

D) EVERYONE =)

Despite a clinic running differently than a floor, no one should be leaving until all urgents are addressed. Regardless of your assignment that day, we all need to ensure the urgent messages are taken care of!

300

True or False:

If I was just confirming a medication with the patient over the phone, I don't need to put a message into Meditech.

False.

Any time you speak with a patient, you should be documenting it in Meditech. As I'm sure we were all taught, if it's not documented, it didn't happen. If it doesn't need to go to anyone else, add it directly to the chart. Document, document, document!

300

True or False:

HaysMed Policy states that cellular devices are not allowed to be carried around by associates.

True!
HaysMed Policy states that phones are to be in lockers, storage, etc. and are not to be in sight.  The policy also states that cellular devices are not to be carried around by associates.
Do not have stricter phone policies put into place!

400

What are 3 top priorities for the CNA each day?

Must include 3 of the following:

-Rooming the patients in the clinic, addressing triage messages/getting it back to the nurse workgroup as soon as possible, urgent messages, infection control/proper hygiene measures

400

True or False:

If I request a day off with sufficient time of notice given by the manager, that day is guaranteed to be off.

False.

HaysMed does have a policy that if the area in which you work would be down too many employees, day off requests can be denied.  There are few times I am unable to make it work, but keep in mind, you must have PTO to cover your days requested.  If you know that you do not have the PTO to cover these days, it is your responsibility to meet with the manager to evaluate if it is still a possibility.

400

True or False:

A message should never be unmarked urgent if it hasn't been resolved, regardless of the reason.

False. If a message is sent back that is marked urgent, but deemed not actually urgent, it is okay to address what you can and unmark as urgent as it continues to be taken care of.  ALWAYS look thoroughly to ensure you are not missing something that actually is urgent.

Describe how to properly use the language line when speaking with the patient.

If instructions for how to use the language line were given, you still get a point, but:

Speak as if you are speaking directly to the patient, not to the interpreter. For example, "Do you still have the cough you had last week?" rather than "Does the patient still have the cough she had last week?"

This may take practice, but it's easy once you get used to it. Patient-centered = better quality care.

400

What should you do if your coworker/friend/family member asks you to look up their lab result/visit/etc.?

Inform them that this is a HIPAA violation, even if the person is requesting it. Advise that they go through the proper channels as it could get you (and them if associated with HaysMed) in serious trouble, including to but not limited to termination, losing certain access/privileges, changing associate responsibilities.

500

True or False:

If I am rooming a patient and realize it hasn't been cleaned, I can continue to room them if it looks clean and/or if the patient who was in there isn't sick.

False. If you get yourself in this position (I have!) you need to proceed to the next clean room and then alert someone to change the room you went to and to ask kindly for them to clean the dirty room

500

True or False:

If a patient being roomed hears my phone conversation with a patient and is able to identify the patient/condition, HIPAA has been violated.

This is technically true.  With our clinic, it is essential to be mindful of this because of where our nursing station is.  Even if unintentional, be mindful of the volume of our voices, the conversations that are happening, and of course try to know when patients are being roomed.  This can be difficult when the nurses' station is full so be mindful!

500

NONURGENT QUESTION =)

Which of the following is a contraindication to spirometry?

A) Previous ICU admissions for asthma

B) Being on home oxygen

C) Lung cancer

D) Pneumothorax

D) Pneumothorax

What are Amanda's dogs and cat names?

Moose, Minnie, and Myles =)

True or False:

Curse words are the only types of profanity that shouldn't take place in the clinic.

False.

This includes, but not limited to speaking poorly on others, using words that are not politically correct, using profanity, and speaking out of frustration.
