Temperature - No higher than 99.5 degrees F

Observe the donor for gait, balance, and speech impediments. Observe the donor for facial asymmetries or facial movements as evidence of an underlying disorder.

Defect must be reported to center management.

Center management must report any defected softgoods to appropriate vendor and utilize the appropriate defect form.

-When donor has 3 consecutive OOL.

-When donor has 4 OOL out of 6 visits.

-When pulse is 125bpm or greater on recheck (or initial if donor does not wish to do recheck).
