As a user of fall arrest equipment in the field; how often should your harness and associated equipment be inspected?

Always inspect your gear before each use.

Annual inspections for recertification are done yearly by Safety Advisors.

100

How large is the "circle of danger" when considering hot work and safe distances to nearby combustibles?

35 feet / 11 metres

100

Name 1 common potential (in our workplace) that could cause fluid injection injury

Hydraulic lines
Grease Guns
Diesel Injection

100

Where would smoke travel underground if a unit caught on fire in the ramp at 2150?

Vent pushes from bottom of mine up.

This would result in all levels above 2150 being compromised. The smoke will exit portals as its main routes of exit. This means both portals (although the vent doors at the upper portal could be closed as the vent tech deems necessary and this would result in all smoke exiting the lower portal where we do not have vent doors).

Always travel down ramp to refuge from a fire location and stay out of compromised air if at all possible and safe to do so.

100

What is the time required being a blasters helper prior to being able to apply for a blasting certificate?

6 months working with a blaster as a helper and has a minimum of 1 years mining experience

200

Specifically underground; how close to an edge that has a fall of greater than 2 metres can you get before you're "at heights"?

3 metres

200

Does the fire watch worker need a radio?

Yes- Calling a Code 1 is the fire watch responsibility

200

Name 3 precautions when working on high pressure lines or tasks

-Do not use hands or body parts to check for leaks
-Always report high pressure fluid leaks to mechanical staff immediately and do not attempt to fix on your own
-Do not handle pressurized lines, connectors, hoses that you suspect could have a high pressure leak
-Stop engine and safely relieve pressure prior to inspecting
-Keep all body parts away from suspected leak areas
-Follow isolation procedures at all times
-Use tools such as steel or wood to check potential leak areas

200

How long would a typical 20lb fire extinguisher last you? (in seconds)

20-25 seconds

200

What is the first sign of CO poisoning you are likely to experience physically in a non-IDLH atmosphere?

Tightness across your forehead

300

What are the two most important duties of the grounds person associated to the working at heights permit?

1- Carry a radio and call a code 1 for every fall regardless of result

2-Understand the approved method of rescue and perform required rescue of the fallen worker

300

How many 20lb (minimum size) extinguishers are required to be on hand for fire watch duties?

1 extinguisher at minimum

300

Why are fluid injection injuries often overlooked or mistaken for other injuries?

Initial injection is not often painful and is usually a small entry wound that looks like a sliver and feels similar. Very little visual cues or identification also results in misclassifications by even highly trained medical professionals.

300

True or False- Even if you think its a coolant line that has blown and is creating smoke...you should always set off the fire suppression system as a precaution

True. Always assume its fire and follow the proper steps of dealing with fire on equipment

1-Set off suppression
2-Call Code 1
3-Isolate unit and put down chocks
4-Use on board fire extinguisher from safe location down wind (if safe to do so)

300

What is a D-Class Fire?

A fire that involves combustible metals such as magnesium, sodium, aluminum and titanium.

400

Name 3 different methods of rescue that are NOT ERT that can be used in the field by a designated grounds person

-Ladder
-Scissor deck unit
-telehandler with basket

400

Name 1 of 2 items that raise the risk of hot work from "High" to "Extreme" risk

1- Hot work inside a building or structure that does not have a functioning fire sprinkler system

2-Hot work performed in a site deemed "critical area"

400

How much pressure is generally required to break the skin and cause an injection injury?

35psi is generally all that is required with close contact

400

What total time will a W65 last during use?

1 hour

400

How do you call a Code-1 from a camp phone?

Dial 4-4-4-4

500

If you are not able to use a self-retracting lanyard (SRL) on your working at heights job, what are two very important portions of the permit to review and understand before working at heights (as the worker at heights)

-Fall Window Calculations
-Fall Protection equipment

All portions are important to review and understand...but understanding your fall window and the calculations associated to hazards below as well as the equipment being used to ensure you have the correct components are essential

500

Name two "critical areas" on site that would result in hot work being an "extreme risk".

Critical Areas/Task Locations on Site:

-Emulsion Building
-Power House
-Fuel Bay or Bulk Fuel Storage
-Server Rooms
-F.A.R or any areas that could jeopardize UG vent functions
-Bulk Lube Storage
-Ammonium Nitrate Building

Tasks:
-Hot Work involving a refuge station UG
-Hot Work within 35 feet of a screen deck Process Plant

500

Fluid Injection injuries require immediate medical attention off site such as imaging and quite likely surgical procedures to be taken to remove fluid from the injection site. If left untreated, amputation and loss of function at minimum is likely going to be a result. 48% of injection injuries result in amputation of the injection location on average.

If left unchecked for over 10 hours...what is the likelihood of potential amputation of the affected body part? (in percentage)

If left untreated, a high pressure injection injury approaches a 100% chance of required amputation of the affected body part.

500
Why is such a small thing such as ensuring the air line is constantly left on in the refuge station when unattended so important in the event of an underground fire emergency situation

If the airline is not on, and the refuge station pressurized, you could walk into a smoke filled refuge station during an emergency

500

Which type of ground support best protects against corrosion ?

Resin anchored rebar
